To protect your privacy, our clinic uses OceanMD for all patient communication—no general email is available. To send documents: call 604-263-2646 and we will provide a secure email link.

Forms: Most forms are not covered by MSP; a fee applies. Forms needing completion by Dr. Modi or Dr. Perez require an in-person appointment.

Appointments & Refills: For questions, medical advice, or prescription refills, an appointment (telephone or in person) is required. Prescription refill appointments should be booked at least two weeks before you run out. Last-minute refill requests cannot be accommodated.
