New Patient Registration

New Patient Eligibility Policy – Please Read Before Registering

To ensure fair access to care for all residents of British Columbia, Dr. Ramin Sariaslani is currently accepting only new patients who are not already attached to a Family Practitioner or Nurse Practitioner in the Lower Mainland.

Please note the following important points before completing the registration form:

If you already have a family doctor or nurse practitioner in the Lower Mainland whose practice is active and has not retired or announced an upcoming retirement, you are not eligible to register at our clinic.

Marpole Medical Clinic does not transfer patients between physicians within the clinic.

We cannot re-attach patients who were previously registered with one of our physicians.

PATIENT INTAKE FORM

To register with Ramin Sariaslan at Marpole Medical Clinic, please follow the steps below.

1. Submit the Online Patient Intake Form

Complete and submit the online patient intake form provided at the bottom of this form.

Important:

Please submit one form per person.

Do not include multiple family member on a single form.

2. Complete Additional Forms

After your intake form has been reviewed, you will receive an email from Marpole Medical Clinic with additional required forms.

Please complete, sign, and return these forms by email.

3. Book a Meet & Greet Appointment

Once all required documents have been received and accepted, you will receive instructions by email on how to schedule a 30–45 minute Meet & Greet appointment with the physician.

During this appointment, you will:

Review your personal and family medical history.

Learn about the clinic’s policies and procedures.

Have the opportunity to ask general questions.

Please note:

Medical concerns will not be addressed during the Meet & Greet appointment. If you have medical issues that require attention, a separate medical appointment must be scheduled.

A Meet & Greet appointment does not guarantee acceptance into the physician’s practice.

Processing time

Registration processing time is approximately 3–4 weeks.

You will receive a confirmation email once your intake form has been reviewed.

*Please do not contact the clinic during the 3–4 week processing period, as this will not speed up processing time. We will reach out if any additional information is needed.
